            1.         I am the record owner or agent for the owner of real property located within Bella Vista Village, more particularly described as (address/Sub-Blk-Lot of property) _________________________

            2.         On ________________________, 20___, I entered into a rental/lease agreement for the lease of said property

with _______________________________________________.

3.         I attest to the following: (choose correct box)

□          The above stated renter/lessee has occupied the above described property for more than 90 days and will continue to do so hereafter.

OR

□          I have entered into a rental/lease agreement with the above stated renter/lessee for the above described property and the terms of said agreement call for a term of not less than ninety (90) days.

4.         I am requesting that the Bella Vista Village Property Owners Association grant

 _______________________________________ “renter guests” membership status based on the above provided information.

5.         I am currently the person responsible for or represent the person responsible for paying the annual and special assessments on the above described property.  I understand all accounts must be current in order for “myself” and my “renter guests” to obtain membership privileges.

6.         My rental/lease agreement with renter/lessee expires on _________________________, 20___.  I understand that any privileges extended pursuant to this request will be valid until the expiration date provided herein, or for one year, whichever date occurs first.

7.         I understand that I can provide a written request to Membership Services asking for said “renter guests” privileges to be withdrawn, if I so choose, at anytime.  

            8.         I have informed my renter/lessee that a “renter guests” membership card must be purchased from the Bella Vista Village Property Owners Association’s Membership Services before access to BVVPOA amenities will be granted.

I swear that all of the above is true.  I further agree to indemnify and hold harmless the Bella Vista Village Property Owners Association (BVVPOA) from any loss which may occur as a result of demands by my “renter guests”.
