WHO: At least one POA
member per team. Each player must have a certified USGA
Handicap. Only one player per team may have a negative
Bella Vista Scramble handicap. Men 70 and older
wishing to play from the No. 3 tees MUST DECLARE THEIR
INTENTION on this form by providing their birth date.
Failure to do so will require the golfer to play from
the No. 2 tees.
WHEN: A shotgun
start at approximately 9 a.m. will be utilized both
days.
WHAT:
Thirty-six holes of scramble format. Three
drives per person per day. Men – blue tees. Men – 70 and
older (must provide birth date information) – white
tees. Women – red tees.
WHERE: Kingsdale
Complex – half of the field will play Kingswood
Saturday/Berksdale Sunday and half will play Berksdale
Saturday/Kingswood Sunday.
Gross and Net Divisions:
All teams compete in both divisions. Fifty percent of
the prize money goes to the gross division and 50
percent of the prize money goes to the net division. A
team can win in only one division, since the highest
possible winnings will determine placement. Forty
percent of the field will share in the prize money.
Skins and closest-to-the-pin prizes will also be
included. All prizes will be gift certificates. Team
handicap to equal a minimum of 0 from table. (Maximum
handicap +8). Golfers not included in the
Bella Vista Handicap System
must have valid copies of handicap cards included with
this form.

Determine each team member’s USGA Slope index from
their handicap card. Assign each player a Bella
Vista scramble handicap based on the tee they will
be playing. Add your Bella Vista scramble handicaps
together to determine your team handicap. Team
handicap must be a minimum of 0.
Entry Fee:
An entry fee of $75 per team must accompany your
application.
** This does not include green fees or carts.
Want to avoid the line at
check-In? Teams may pre-pay green fees and/or
cart fees for the two-day tournament. Pre-payment must
be for the entire team.
All registrations and cancellations must be made by
Friday March 23, 2012.
